Information
Please call our office at 305.479.2973 to schedule your initial appointment. Be sure to have your health insurance information handy, so we can verify your benefits and eligibility prior to starting. On the day of your appointment, please try to arrive to the office fifteen minutes early so you can fill out all necessary forms prior to examination. Please bring any and all information regarding your injury, including but not limited to, a physical therapy prescription, X-ray or MRI reports, medication lists, etc.
If you are coming in for treatment of injuries resulting from an accident (auto accident, slip/fall, Worker’s Compensation, etc.), you will need to fill out forms in the office required by Florida Law.
Once you are in our system, our system will send appointment reminders by either call, email, or text message.
